How to get a referral: Information for patients.

  • If you have cataracts and meet the criteria for NHS surgery your optometrist will discuss this with you and offer you referral for surgery. At this point if you wish to be seen by BHA, then simply ask your optometrist to refer you direct to us. Once we have received your referral, we will contact you to arrange your assessment and surgery.
  • If you have been referred to another provider – but now wish to be referred to BHA – then simply contact the optometry practice that referred you and request they refer you to us instead. They will forward your referral to us and once we have received it we will contact you to arrange surgery.
  • Can I self-refer? No, sadly within the NHS setting, you will need a referral from your optometrist, your GP or a hospital specialist.

  • Herefordshire optometrists with access to Pharmaoutcomes may refer via this portal by simply selecting BHA in the dropdown menu in the pre-cat assessment referral page.
  • Referrals can also be made securely by clicking here.
  • Alternatively, if you wish to generate a referral using your electronic patient record and refer using NHS.net or equivalent secure email then forward the referral to hwicb.referrals.bha@nhs.net
